You can register at the Practice by completing an application form available from reception. Please complete all the details on the form – doing so will speed up the application process. You do not need proof of address or immigration status, ID or an NHS number.
You will be invited to attend a New Patient Check with a Practice Nurse. This check needs to be completed before you can make an appointment with a doctor. You are registered with the Practice, rather than with an individual GP. However, you may request to be seen by the practitioner of your choice for the purposes of continuing care, care of particular conditions, gender or ethnicity. If you wish to exercise this right, you may have to wait longer to see your preferred practitioner.
The practice is required to provide you with a named GP who will have overall responsibility for the care and support that our surgery provides to you. Your named GP will also work with other relevant health and care professionals involved in your care to ensure that your care package meets your individual needs. If you wish to know who your named GP is, please ask a member of staff.
This does not prevent you from seeing any GP in the practice.
